Lofnac-100 is a rectal suppository containing diclofenac sodium, a non-steroidal anti-inflammatory drug (NSAID). It contains 100 mg of diclofenac sodium and various excipients.
Lofnac-100 inhibits the enzyme cyclooxygenase, reducing prostaglandin production and alleviating pain, inflammation, and fever.
Lofnac-100 is absorbed rapidly rectally, reaching peak plasma concentrations within 1 hour. It is extensively bound to plasma proteins and eliminated through urine and feces.
Lofnac-100 is indicated for pain and inflammation relief in various conditions, including arthritis, gout, chronic polyarthritis, Bechterew's disease, inflammatory rheumatoid disorders, degenerative joint disorders, soft tissue rheumatism, and post-injury/surgery inflammation.
**Adults:** 75-150 mg daily, divided into multiple doses, maximum 150 mg daily. **Children:** Lofnac 12.5/25 is preferred. 1-3 mg/kg per day for juvenile chronic arthritis, 1-2 mg/kg per day for acute post-operative pain (max 4 days).
Lofnac-100 suppositories should be inserted rectally after bowel clearance, following instructions in the text.
Lofnac-100 is contraindicated in cases of hypersensitivity, active gastrointestinal ulcers, history of NSAID-related bleeding, active peptic ulcer/haemorrhage, last trimester pregnancy, severe hepatic/renal/cardiac failure, asthma/angioedema/urticaria triggered by NSAIDs, proctitis, and established congestive heart failure.
Caution is advised for elderly patients, patients with gastrointestinal disorders, history of ulceration/bleeding, impaired hepatic/renal function, and systemic lupus erythematosus.
Common side effects include headache, dizziness, increased transaminases, nausea, vomiting, diarrhea, and rash. Rare or very rare side effects include anemia, agranulocytosis, hypersensitivity, edema, depression, disturbances, hallucinations, and many others.
Lofnac-100 should be used cautiously with lithium, digoxin, diuretics, anti-hypertensives, antibacterial, phenytoin, colestipol, cholestyramine, cardiac glycosides, potent CYP2C9 inhibitors, glucocorticoids, methotrexate, acetyl-salicylic acid, other NSAIDs, SSRIs, cyclosporin, tacrolimus, drugs causing hyperkalemia, and quinolone. Avoid use for 8-12 days after mifepristone administration.
Lofnac-100 is contraindicated in the last trimester of pregnancy. It passes into breast milk and should not be administered during pregnancy or breastfeeding.
Overdosage can cause various symptoms like headache, nausea, vomiting, gastrointestinal bleeding, dizziness, disorientation, excitation, coma, drowsiness, tinnitus, fainting, or convulsions. In case of significant poisoning, acute renal failure and liver damage are possible. Seek immediate medical attention.
