Amalar is used for intermittent preventive treatment (IPT) of malaria in pregnant women during the second and third trimesters.

2. What is the mechanism of action of Amalar?

Amalar contains sulfadoxine and pyrimethamine, which are folic acid antagonists. They disrupt the parasitic life cycle by inhibiting dihydropteroate synthase and dihydrofolate reductase, respectively.

3. What are the serious side effects associated with Amalar?

Serious side effects include Stevens-Johnson syndrome, toxic epidermal necrolysis, blood disorders, active bacterial or fungal infections, and allergic reactions.

4. What are the ingredients in Amalar?

Each Amalar tablet contains 500 mg of sulfadoxine and 25 mg of pyrimethamine.

5. How should Amalar be taken for IPT in pregnancy?

Three tablets are taken at once, one month apart, during the second and third trimesters. At least three doses should be given, with the last dose one month before the expected delivery date. It should not be used in the first trimester.

6. What are the contraindications for Amalar use?

Contraindications include inadequate folic acid levels, bone marrow dysfunction, anemia, liver or severe renal impairment, seizures, G6PD deficiency, porphyria, hypersensitivity to sulfonamides or pyrimethamine, severe hepatic damage, confirmed renal impairment, history of skin allergies, and history of blood dyscrasias or bone marrow depression.

7. Are there any drug interactions with Amalar?

Concomitant use with chloroquine may increase adverse reactions. Antifolic drugs should not be used with Amalar. While compatible with quinine and antibiotics, inform your physician of all medications.

8. What should patients be advised about regarding Amalar?

Patients should be advised about potential serious side effects and to seek immediate medical attention if they occur. They should maintain adequate fluid intake, watch for warning signs, understand Amalar is not 100% effective, and seek medical attention for any fever after returning from a malarious area.

9. What should be done in case of an Amalar overdose?

Induce vomiting and gastric lavage, followed by purges. Maintain adequate hydration. Monitor renal, hepatic, and hematopoietic systems. Treat seizures with diazepam or a barbiturate. Administer folinic acid for low platelet or white blood cell counts.

10. What are the pharmacokinetics of Amalar?

Amalar is well-absorbed, reaching peak plasma concentrations in about 4 hours. Both ingredients are highly protein-bound, cross the placental barrier, and are eliminated primarily via the kidneys. Sulfadoxine has a half-life of around 200 hours, and pyrimethamine's is approximately 100 hours.
