What is the active ingredient in Ovumine 50?
Clomiphene citrate USP, 50mg per tablet.
How does Clomiphene citrate work?
It acts as both an estrogen and an anti-estrogen, stimulating the pituitary gland to release gonadotrophic hormones. This leads to increased gametogenesis (egg production) and steroidogenesis (hormone production) in the ovaries, ultimately resulting in ovulation.
How is Clomiphene citrate absorbed and eliminated from the body?
It's well absorbed after oral administration and eliminated slowly from the plasma, with a half-life of 5-7 days. This slow elimination is due to its high affinity for plasma proteins, enterohepatic re-circulation (passing through the liver and intestines multiple times), and accumulation in fatty tissues.
What happens to Clomiphene citrate in the body?
It's metabolized into derivatives that also have anti-estrogen activity.
What conditions is Ovumine 50 used to treat?
It's used to treat anovulation (lack of ovulation) and oligo-ovulation (infrequent ovulation) in women desiring pregnancy, provided their hypothalamic-hypophyseal-ovarian system is functional and they have adequate endogenous estrogen.
Who should not take Ovumine 50?
It's contraindicated in: - Pregnant women - Patients with liver disease or history of liver dysfunction - Endometrial carcinoma - Ovarian cysts (except polycystic ovary syndrome) - Undiagnosed abnormal uterine bleeding - Pituitary failure - Hypersensitivity to clomiphene - Active thrombophlebitis - Mental depression
What is the recommended starting dose of Ovumine 50?
1 tablet (50mg) daily for 5 days.
When should therapy be started?
It can be started at any time if the patient hasn't had recent bleeding. If progestin-induced bleeding is planned or spontaneous bleeding occurs before therapy, the regimen should be started on or about the fifth day of the cycle.
Can the dosage be increased if ovulation doesn't occur in the first cycle?
If ovulation doesn't occur with the first course, a second course of 100mg daily (two tablets) for 5 days can be started as early as 30 days after the previous one.
What is considered an adequate trial of clomiphene therapy?
Three courses of therapy are considered an adequate trial. If ovulation hasn't occurred after this, the diagnosis should be reevaluated.
When does ovulation usually occur after a course of Ovumine 50?
From days 6-12 after a course.
How long should therapy be continued?
Most patients will ovulate after 3 courses. Long-term use is not recommended, exceeding 6 cycles (3 ovulatory cycles) is not advised.
What are the most common side effects of Ovumine 50?
Reversible ovarian enlargement and cyst formation, vasomotor flushes (resembling menopausal symptoms), abdominal or pelvic discomfort or pain sometimes with nausea or vomiting.
What other side effects have been reported?
Breast tenderness, abnormal uterine bleeding, weight gain, headache, increased urinary flow, transient eye disturbances (blurring, spots, flashes), cataracts (rare), allergic rashes, urticaria, reversible hair loss, hydatidiform mole formation, syncope/fainting, cerebrovascular accident, central thrombosis, psychotic reactions, neurological impairment, disorientation, speech disturbance, dizziness, lightheadedness, nervous tension, fatigue, vertigo, insomnia, depression, abnormalities in liver function tests.
What is the risk associated with Ovumine 50 use?
Increased risk of multiple births.
What should patients be warned about regarding visual symptoms?
Visual symptoms can make driving or operating machinery more hazardous, especially in poor lighting. If visual symptoms occur, Ovumine 50 should be discontinued and a complete ophthalmological evaluation performed. No further courses should be administered.
What should patients be advised to report?
Any abdominal or pelvic pain as it may indicate ovarian cyst formation or enlargement. If abnormal enlargement occurs, clomiphene citrate should not be administered until the ovaries return to their pre-treatment size.
How is ovarian enlargement typically managed?
It usually regresses spontaneously within a few days or weeks after discontinuing treatment and should be managed conservatively. The dosage and/or duration of the next dose may be adjusted.
What precautions should be taken for patients with polycystic ovaries?
They should receive the lowest possible doses to prevent further enlargement or cyst formation. Prolonged courses of treatment are not recommended.
Is Ovumine 50 safe for breastfeeding women?
Safety during lactation hasn't been established.
What are the signs and symptoms of Ovumine 50 overdose?
Nausea, vomiting, vasomotor flushes, visual blurring or flashes, scotomata, ovarian enlargement with pelvic or abdominal pain.
How is Ovumine 50 overdose treated?
Gastric lavage is recommended. Further treatment is supportive and symptomatic. Female patients who are capable of reproduction should be observed for 2-3 weeks in case of ovarian enlargement.

It's packaged in a blister of 10 tablets in a carton. Store below 25°C, protect from light, and keep out of reach of children.
